The Day After 4/20 - National Surprise Drug Test Day

Dr. Matthew Nuesse

Medically reviewed by Dr. Matthew Nuesse, Certified Medical & DOT Examiner

With April 20 around the corner, cannabis enjoyers are getting ready to celebrate. And it’s a carefree day that brings THC enthusiasts together. But, the very next day, for employed people, there comes a dark (and to a degree controversial) twist.

While it’s not official, National Surprise Drug Test Day is real, and it’s affecting people, both people who partake in 4/20, and those who decided to quit some time before it. The problem isn’t 4/20, it’s not knowing where you stand on 4/21.

Given that people’s jobs are at stake, let’s talk about drug testing and THC metabolism to shed some light on the topic and avoid the most common mistakes people make.

A Brief History of 420

The origin story of 4/20 is tied to a group of high schoolers from San Rafael High School, California, back in 1971. They would meet after school at 4:20 pm to smoke up and look for a mysterious cannabis patch of land (that was never found).

But the spreading of the term “420” was amplified two decades later, thanks to the popularity of The Grateful Dead. By chance, the brother of one of those high-schoolers got into the inner circle of the band, and the phrase started spreading, resulting in the status it has today.

National Surprise Drug Test Day - The Day After

The ethics of conducting a drug test the day after 4/20 are questionable. That’s why the National Surprise Drug Test Day isn’t actually official, similar to 4/20 itself.

But it does happen, through the lens of random drug testing, which is legal and is conducted without timely notice. That’s how employers can justify employment drug testing just after 4/20.

Have a look at what our Certified Medical & DOT Examiner, Dr. Matthew Nuesse, has to say on National Surprise Drug Test day:

Random drug testing in the workplace

When it comes to employment drug testing, there are several types or causes for it, with random drug testing being among them.

The key thing about random drug testing, though, is that it doesn’t have to be announced in advance. It can even be the same day. Testing schedules can sometimes coincide with dates like April 21st, but random testing, by definition, isn’t supposed to target specific events. Reasonable suspicion drug testing has different regulations, and it’s handled with more privacy, as well as timely scheduling.

Random drug testing is mostly done through urine samples, several times per year. Now, the employees are selected in advance, and there’s no guarantee that the same people won’t get tested more than once. On the other hand, some employees won’t be tested at all in a single year, which is why the random factor is so strong in this type of drug testing.

What are the consequences after a failed drug test

If you fail a urine drug test at work, several things could happen, depending on the company policy:

  • Job termination
  • Disciplinary action (suspension from work)
  • Counseling and rehabilitation (“return-to-duty” process)

Now, the actions after a failed drug test heavily depend on the industry you’re working in:

  • Federally regulated jobs (like trucking, aviation):
    • Strict rules, often mandatory removal from duties
  • Safety-sensitive roles (construction, manufacturing, healthcare):
    • Higher likelihood of termination
  • Office or non-safety roles:

Legal does not equal a THC test pass

One mistake cannabis enjoyers often make is thinking that legality in their state = THC test pass. This is, at least for now, false.

In companies that are not federally regulated, there might be a way to justify THC use with the proper documentation, at least for people who are medically prescribed some form of cannabis product.

However, in most cases, legality does not give you a free pass on a THC urine drug test, meaning that you shouldn’t rely on the legal status of cannabis in your area.

Factors That Affect THC Metabolism

Now that we know what could happen the day after 420, let’s talk about how THC acts throughout the metabolism and what affects its elimination. Before we get into individual factors, let’s quickly take a look at how THC “travels” throughout the body.

In simple terms, the four stages of THC metabolism are:

  1. Absorption - smoking, vaping, and eating edibles
  2. Distribution - THC travels to the brain, and some of it gets stored in fat tissues
  3. Liver metabolism - the liver breaks down THC into 11-hydroxy-THC, which is still psychoactive, and THC-COOH (a metabolite that drug tests are designed to detect)
  4. Elimination - the majority of THC is eliminated through feces and urine

The biggest issue with THC is that it’s lipophilic, meaning that any extra amount that the liver cannot process gets stored in body fat. This can affect drug test results days, weeks, or even months after you stop using. That’s why these factors are crucial to getting clean:

  • Age - the older we get, the slower and less potent our metabolism is
  • Body-fat percentage - people with a higher body-fat content have more storage room for THC
  • Frequency and dosage - people who use more often and/or more potent THC products store more THC in their fat cells
  • Hydration and diet - proper hydration helps with speeding up the metabolism, and a fiber-rich diet helps with THC elimination
  • Physical activity - active people have generally faster metabolism, which translates to faster THC elimination directly

So, how long does THC stick around in your body?

Considering all the mentioned factors, there is no strict formula to predict the exact moment when you’ll get clean. On top of that, THC tends to stick for way longer than other substances.

Here’s a general THC detection time estimate to put things in perspective:

Test Type Occasional Use (1–2x/week) Moderate Use (few times/week) Heavy Use (daily)
Urine Test 1–3 days 5–10 days 10–30+ days
Saliva Test 24–48 hours 1–3 days 1–7 days
Blood Test 1–2 days 2–7 days Up to 7+ days
Hair Test Up to 90 days Up to 90 days Up to 90 days

Because these timelines vary so much, we designed a THC Detection Calculator to help you get a better understanding of where you stand.

What you can do to speed up THC detox

Speeding up the THC detox process is closely tied to the factors that affect its accumulation in the body:

  • Stop using ASAP, especially if you have a drug test coming up (possibly right after 420)
  • Start exercising to speed up your metabolism
  • Increase your hydration, but don’t overdo it (modern lab tests detect urine dilution)
  • Introduce fiber-rich fruit and veggies to your diet to help with THC elimination
  • Get a detox supplement to help your liver with THC metabolism (not an instant solution, but it helps a great deal)
  • Test at home with a 50 ng/ml cutoff test, or a 3-level THC urine test if you’re a heavy user

What doesn’t work when anticipating a THC urine drug test

People have tried to cheat on a THC urine drug test in various ways. To not fall into a trap, it’s important to be aware of the myths circling the internet. Most popular things that don’t exactly work include:

  • Drinking excessive amounts of liquid - urine overdiluting can be detected with modern lab tests
  • Sauna and exercise right before the test - regular exercise may support overall metabolism over time. However, exercising right before a test can temporarily increase THC levels in urine
  • Home remedies as magical detox potions - While drinks like cranberry juice may support hydration, they do not meaningfully speed up THC elimination
  • Waiting a few days - unfortunately, the fat-soluble nature of THC works against you here
  • Thinking “all is good” because you haven’t smoked for a few days - this is way too common. It’s best if you test at home to see how things really stand, instead of guessing

4/20 and 4/21 Drug Testing - FAQ

1. Will I pass a drug test if I smoked on 420?

If you smoked, vaped, or took an edible on 420, the chances are high that you would test positive on April 21st. If you’re expecting a random drug test at your workplace, it’s best not to risk it.

Even for one-time smokers, THC can still be detected in urine for a few days

2. How long does THC stay in urine?

It depends on metabolic factors and usage habits. The general timeline for THC detection in urine is:

  • Occasional users (1–2 times):
    • 1–3 days
  • Light users (a few times per week):
    • 3–7 days
  • Moderate users (regular weekly use):
    • 7–14 days
  • Heavy users (daily use):
    • 10–30+ days

So, even if 420 is the only day you plan to partake, on 4/21, you will likely test positive.

3. Is 30 days enough to get clean from THC?

If you're an occasional user (once or twice per week), and you stopped today, 30 days should be enough to get clean from THC.

But, if you use more frequently, especially products on the potent side, this can extend way past 30 days, especially if you have a high body-fat percentage.

There is no fixed timeline for getting clean from THC due to various factors like metabolism and usage habits.

4/20 Can Be Fun, But 4/21 Not so Much

Unfortunately, there’s no way of knowing whether your employer will conduct a test the day after 420. If you have any suspicions about it, the ideal scenario is not only not to partake in it, but to stop using a month before April 21st comes around.

So, keep the THC detox factors in mind, avoid falling for myths, test at home, and give your body enough time to metabolize and eliminate any potential THC buildups.
